agxv: Assign set for new descriptor pool entries in agxv_descriptor_set_create
Was causing crashes when using vkResetDescriptorPool.
Found with "dEQP-VK.api.descriptor_pool.repeated_short" test. (and fixes dEQP-VK.api.descriptor_pool.repeated_*)
