Update documentation (and rename suffixed_image)

Turns out 324c7871 didn't actually rename the suffixed image, so this one is still inconsistent. It's renamed now but that's an API break, cc @anholt, @daenzer.

The rest is just documentation, adding the previously missing FDO_ variables, switching to ..attribute:: for better rendering and some other bits and bobs.
