rtphdrext-rfc6464: Add example, test and don't fail when used with the level element.

  • Add test on inserting the extension based on the caps.
  • Add an example pipeline in the doc, to make it easier for the next one

And also fix a bug where output of our "level" elements caused it to not insert a level if the number if too high.
