Changes:

Fixed description for FastTracker II instrument files (they were incorrectly called Scream Tracker instrument files).
Added Scream Tracker 3 instrument files.
Added Impulse Tracker instrument files
Added OpenMPT module files.
Added OpenMPT custom tuning files.

Hopefully I didn't mess anything up. Let me know if I did anything wrong.
