Update URLs to reflect gitlab migration

Updates bug & patch submission info in README.md, configure.ac, and amdgpu.man.

While doing this I also noticed that amdgpu.man sends people to irc.freenode.net still, but as that's still the listed IRC home on https://www.x.org/wiki/radeon/ I left it for now.
