Update README & configure.ac for gitlab migration

Moves README to README.md so that gitlab displays with markdown formatting, and updates contents with gitlab URLs.

Also adds a pointer to the main xcb website, and updates the description slightly to reflect the current relationship between Xlib & XCB a little better.
